1CHANNEL 1 · AGENCY AND WAIVER ENROLLMENT
State funds are allocated to HCBS waiver programs. A care coordinator refers the member. Funds are preauthorized at the 1915(c) waiver rate. An IDD engagement coach meets the member, reviews the individual support plan (ISP), and confirms goals. Services are delivered on the platform. Billing recurs monthly while the plan is active.
The care coordinator at the DSP identifies and refers the individual for Hello, It's Me® services aligned to the Medicaid HCBS waiver.
Our IDD engagement coach contacts the member and schedules an intake meeting.
Coach and member review the individual support plan (ISP) and personal goals together.
The coach creates a personalized support plan that documents goals and progress, used as evidence by the agency and care coordinator.
2CHANNEL 2 · DIRECT APP ACCESS
Members download the app for structured, safe social connection and life-skills content. Coaching is available only through enrolled care plans.
