HIPAA Notice of Privacy Practices
Hello, It's Me® handles protected health information when we deliver Medicaid-funded services. When we do, we act as a business associate of the agencies and providers we work with, and we follow the Health Insurance Portability and Accountability Act, known as HIPAA. This notice explains how we may use and share your protected health information and the rights you have over it.
What protected health information is
Protected health information is information that identifies you and relates to your health, your care, or the payment for your care. Examples include your support plan goals, your coaching records, and the services billed on your behalf.
How we may use and share your information
For treatment and support
We use your information to deliver coaching and support aligned to your individual support plan, and we share it with your coach, your care coordinator, and the agency that supports you, so they can help you reach your goals.
For payment
We use and share your information so that Medicaid programs and the agencies we work with can authorize and pay for your services.
For health care operations
We use your information to run the platform safely and well, including quality review, training, and documenting outcomes for the agencies that enroll you.
When the law requires or permits it
- To public health and safety authorities, when the law requires it.
- To protect you or another person from serious harm.
- To comply with a court order, subpoena, or other lawful request.
- For oversight activities that the law authorizes.
Uses that need your written permission
Most uses and disclosures beyond those described above require your written authorization. We will never sell your protected health information, and we will not use it for marketing without your written permission. If you give us authorization, you can take it back in writing at any time, and we will stop the use going forward.
Your rights over your health information
- You can ask to see and get a copy of your protected health information.
- You can ask us to correct information you believe is wrong or incomplete.
- You can ask us to limit how we use or share your information.
- You can ask us to contact you in a specific way or at a specific place.
- You can ask for a list of certain disclosures we have made.
- You can get a paper copy of this notice at any time, even if you agreed to receive it electronically.
- You can file a complaint if you believe your privacy rights have been violated.
Our responsibilities
We are required by law to protect the privacy of your protected health information, to follow the duties and privacy practices described in this notice, and to tell you promptly if a breach occurs that may have compromised the privacy or security of your information. We will not use or share your information other than as described here unless you tell us in writing that we can.
Changes to this notice
We can change this notice, and the changes will apply to all the information we hold. The new notice will be available on request, on our website, and at our office. We will post the new effective date when we make a change.
How to exercise your rights or file a complaint
To exercise any right in this notice, or to file a complaint, contact our Privacy Officer. You will not be penalized for filing a complaint.
